Getting Started

Call the Child Development Center at 973-971-5227 and provide us with the child’s name, address, county, name of parent/guardian and phone information, insurance name, and referral source.

The intake worker will return your call and discuss your child’s evaluation with you. To expedite the process, please print and complete our Childhood History Form; our clinicians use this information to assist them in evaluating your child.
