Parking: Visitor Parking Lot - Madison Avenue Entrance

Special Instructions: Patients must bring ID and insurance card(s). A signed prescription is required for all procedures. It must include the proper diagnosis code (ICD-9 code). No appointment is necessary.

Please note: If you have a scheduled appointment in another department, you must arrive a minimum of 30 minutes prior to your appointment time so you can be registered first.
