

Great medical care begins with your first phone call to the Atlantic Health System Pulmonary Center and continues even after you leave the hospital. Social workers and nursing care managers in our continuum of care departments assist pulmonary patients and families by identifying ongoing health care services, transitional services and community resources that can help the patient once he or she is discharged. These services may include home care, skilled facility care, rehabilitation, medical equipment and hospice.
Social workers also provide counseling to help patients and their families learn to cope with health care challenges.
