Atlantic Home Care is a Medicare and Medicaid certified home health agency providing skilled nursing and rehabilitation therapy to patients in Morris, Union, Essex and Somerset counties in New Jersey and Pike County in Pennsylvania.
Our home care team works closely with Atlantic Health System Pulmonary Center physicians, patients and their families to develop a plan of care to meet each patient's unique care needs. Whether recovering from pneumonia, coping with cardiac or pulmonary disease or improving function impaired by a stroke, Atlantic Home Care helps patients remain in their homes and enhances their quality of life.
Your doctor may prescribe home care to:

Chronic Obstructive Pulmonary Disease (COPD) Management
The COPD Disease Management Program, offered by Atlantic Home Care, combines a multidisciplinary approach to COPD management with TeleHealth, technology that monitors each patient's status on a daily basis and makes health care recommendations based on its measurements, including appropriate medications, environment factors that may be triggering flare-ups, recommended exercises and more. Our COPD management program provides greater continuity of care and communication between caregivers, which can reduce emergency department visits and unplanned hospitalizations.
