Pre-Procedure Evaluation
Pre-procedure testing is performed at 111 Madison Avenue, Fourth Floor, Morristown, NJ. All testing must be scheduled by calling 973-971-6800. Our hours of operation are Monday through Friday 7:00am to 4:00pm, and Saturday 7:00am to 12:30pm. We are closed on Sunday.
When you come to your appointment, bring your identification and insurance card(s) with you, even if you have already pre-registered over the phone. If you have an advance directive, please bring a copy. You will be registered under the name on your license or passport – not the name on your insurance card or credit card. A signed prescription with the proper diagnosis code (ICD-9) is required for each procedure.
