Mental Health Appointments
The first step in making a mental health appointment with the Developmental Disabilities Centers (DDC) at Morristown Medical Center and Overlook Medical Center's Union Campus is to discuss your needs with one of our staff members. You will then receive in the mail a packet that includes an initial intake form, outpatient consent for treatment and a release of records form. We must receive completed intake forms, plus a copy of your insurance card, proof of guardianship, social history and psychological evaluation before we can schedule an appointment.
Please bring the following items to your mental health appointment:
- A list of the medications you are currently taking
- Notations of any medical or mental health changes since your previous appointment
- Past mental health records, including:
- Chronic illness
- Family history of emotional or psychiatric illness
- Psychiatric or medical hospitalizations
- Psychiatric or psychological evaluations
- Social history
- Surgeries
- The name and phone number of your pharmacy
- Your current insurance card
- Free and personalized
- Choose health topics that matter most to you and your family
- Learn about programs, screenings and support groups
