How To Register
Before arriving at Morristown Medical Center or Overlook Medical Center, you must obtain a registration packet from your physician or the hospital, complete the form and send it in. Each time you plan to deliver a baby, you need to re-register.
If you would like to obtain newborn metabolic screening in addition to what is required by the State of New Jersey, you will also need to contact a private company to make arrangements in advance. Your registration packet includes information on the testing, for which there is an additional fee. You must send the signed form back with the registration that confirms you are aware that additional testing is available for a fee through private companies.
Also included in the registration packet are a Notice of Privacy and an optional advanced directive form.
When you return your registration information, please include:
- Your completed registration form
- A copy of your insurance
- A copy of a form of ID (driver's license)
- A signed “Acknowledgement of Receipt of Notice of Availability of Supplemental Screening “ form
- Advanced directive (optional)
