Frequently asked questions

Medical records can be requested, received, and reviewed all in one place – your MyChart patient portal. Records from hospital and Atlantic Medical Group doctor visits are available there, along with tools for prescription refills, bill pay, and appointment scheduling.

In addition, hospital records can be requested by filling out an authorization form. Forms can be delivered, mailed, faxed, or scanned and emailed. Providers who don't participate in MyChart can be given temporary access to your health records.

Our medical centers and Atlantic Medical Group physicians are in network with many insurance providers. Patients should always check with their individual carriers to determine our participation with their plans.

For most hospital services

To request, cancel or reschedule an appointment for most hospital services, call our Atlantic Health Central Scheduling Department Monday through Friday, 8:00am to 6:00pm, for assistance 1-844-264-2250. Please have your script, ICD-10 code and insurance information ready. For CentraState Central Scheduling please call 732-294-2778.
