How to register for child birth

Preparing for your baby's arrival starts with hospital registration. Learn how to pre-register, submit required documents and complete important steps before delivery.

Complete childbirth registration

Before arriving at an Atlantic Health maternity center, you must obtain a registration packet from your physician or the hospital, complete the form and send it in at least eight weeks prior to your delivery date. Each time you plan to deliver a baby, you need to re-register.

If you would like to obtain newborn metabolic screening in addition to what is required by the State of New Jersey, you will also need to contact a private company to make arrangements in advance. Your registration packet includes information on the testing, for which there is an additional fee. You must send the signed form back with the registration that confirms you are aware that additional testing is available for a fee through private companies.

When you return your pre-registration information, please include:

  • Your completed pre-registration form
  • A copy of your insurance
  • A copy of a form of ID (driver's license)
  • A signed “Acknowledgement of Receipt of Notice of Availability of Supplemental Screening“ form

A notice of privacy and an optional advanced directive form will be provided at labor and delivery.
