Proxy access allows parents, legal guardians and caregivers to log into their personal MyChart account and connect to the health information of a patient they represent. Users can communicate with the doctor’s office and enjoy other MyChart benefits, like managing appointments online and reviewing test results. All that’s needed is a completed proxy authorization form accompanied by a government-issued photo identification, such as a driver’s license, passport or non-driver ID.

What is proxy access?
Proxy access allows parents, legal guardians, and caregivers to log in to their own MyChart account and connect to medical information for the patient they represent.
The level of access available depends on the patient’s age and required authorization.
MyChart for patients
With MyChart, patients can:
- Access test results
- Communicate with their doctor’s office
- Manage appointments
- Request prescription refills
- View and pay hospital bills
- View clinical notes
MyChart for proxies
Proxy access lets an authorized individual help manage a patient’s care in MyChart. The features available depend on whether the proxy has Partial Access or Full Access.
Partial proxy access
With partial access, proxies can:
- See most information the patient can view, excluding sensitive topics
- Communicate with the doctor’s office
Proxies cannot:
- Request prescription refills
- View appointments
- View clinical notes
- View medication information
Full proxy access
With Full Access, proxies have the same features and benefits as the patient, including:
- Access to test results
- Communication with the doctor’s office
- Appointment management
- Prescription refill requests
- Access to clinical notes
Proxy access by age group
Patients age 11 and under
- Children do not need their own MyChart account
- Partial proxy access does not apply to this age group
- Full proxy access is available to parents and legal guardians
- For patients ages 0–11, clinical notes are not available in MyChart
Patients ages 12–17
- Patients can discuss whether MyChart is appropriate at their next doctor’s visit
- Proxy access becomes limited starting on the patient’s 12th birthday to protect patient privacy
- With patient authorization and agreement from a trusted care provider, full proxy access may be granted
- Proxy access is reassessed annually
- Proxy access ends on the patient’s 18th birthday
Patients age 18 and over
- Patients can create and manage their own MyChart account
- Partial proxy access does not apply to this age group
- Adult‑to‑adult proxy access is available with patient authorization
