Leaving the hospital & care after your visit

When leaving the hospital, patients will receive essential information, including post-treatment care instructions, medication details and follow-up appointments. This will help ensure a smooth transition and continued recovery at home.

Before going home from the hospital

When the person who will take you home has arrived:

Notify your nurse
Decide with your nurse whether or not you will need a wheelchair to leave your hospital room
Check all closets, bathrooms, nightstands and tray tables for your belongings; the hospital cannot be responsible for personal items left behind
Retrieve all valuables and medications from the security and pharmacy departments, if applicable

Patients recovering from surgery or an injury

For some patients, inpatient rehabilitation can help them regain independence before transitioning home

After returning home from the hospital setting, you may need outpatient rehabilitation therapeutic services
