Leaving the hospital & care after your visit
When leaving the hospital, patients will receive essential information, including post-treatment care instructions, medication details and follow-up appointments. This will help ensure a smooth transition and continued recovery at home.

Before going home from the hospital
When the person who will take you home has arrived:
- Notify your nurse
- Decide with your nurse whether or not you will need a wheelchair to leave your hospital room
- Check all closets, bathrooms, nightstands and tray tables for your belongings; the hospital cannot be responsible for personal items left behind
- Retrieve all valuables and medications from the security and pharmacy departments, if applicable
Patients recovering from surgery or an injury
- For some patients, inpatient rehabilitation can help them regain independence before transitioning home
- For those recovering from a major injury, surgery or illness, each of Atlantic Health's hospitals – provide expert acute care inpatient rehabilitation services
- After returning home from the hospital setting, you may need outpatient rehabilitation therapeutic services
