CentraState Medical Center earns national recognition from the American Heart Association for high-quality care in heart failure and stroke

The American Heart Association presents the Get With The Guidelines® awards for proven dedication to best practices and life-saving care

FREEHOLD, NJ - AUGUST 20, 2025 - CentraState Medical Center has received two American Heart Association Get With The Guidelines® achievement awards, recognizing the hospital's commitment to following up-to-date, research-based guidelines for the treatment of heart disease and stroke, ultimately leading to more lives saved, shorter recovery times and fewer readmissions to the hospital.

Heart disease and stroke are the No. 1 and No. 5 causes of death in the United States, respectively, according to the American Heart Association. Studies show patients can recover better when providers consistently follow treatment guidelines.

This year, CentraState was recognized with the following awards:

  • Get With The Guidelines®-Heart Failure Gold Plus quality achievement award for a commitment to improving outcomes for patients with heart failure, meaning reduced readmissions and more healthy days at home.
  • Target: Heart FailureSM Optimal: Hospitals receiving this meet specific criteria that improve medication adherence, provide early follow-up care and coordination, enhance patient education and have performed 50% or better for defect-free care. The goal is to help patients improve their quality of life in managing this chronic condition.
  • Target: Type 2 Diabetes Honor Roll™: Hospitals aim to ensure patients with Type 2 diabetes, who might be at higher risk for complications, receive the most up-to-date, evidence-based care when hospitalized due to heart disease or stroke.
  • Get With The Guidelines® – Stroke Gold Plus award for proven dedication to ensuring all stroke patients have access to best practices and life-saving care.
  • Target: StrokeSM Honor Roll Elite: Hospitals must meet specific criteria that reduce the time between an eligible patient's arrival at the hospital and treatment with thrombolytic therapy.
  • Target: Type 2 Diabetes Honor Roll™: Hospitals aim to ensure patients with Type 2 diabetes, who might be at higher risk for complications, receive the most up-to-date, evidence-based care when hospitalized due to stroke.

Get With The Guidelines puts the expertise of the American Heart Association and American Stroke Association to work for hospitals nationwide, helping ensure patient care is aligned with the latest research- and evidence-based guidelines. As a participant in the Get With The Guidelines, CentraState qualified for the awards by demonstrating how their organization has committed to improving quality care.
