Where can I find information about COVID-19 vaccines and testing?

How can I get medical records?

Medical records can be requested, received, and reviewed all in one place – your MyChart patient portal. Records from hospital and Atlantic Medical Group doctor visits are available there, along with tools for prescription refills, bill pay, and appointment scheduling.

In addition, hospital records can be requested by filling out an authorization form. Forms can be delivered, mailed, faxed, or scanned and emailed. Providers who don't participate in MyChart can be given temporary access to your health records.

How can I get a copy of a birth certificate?

After it is completed at the hospital, the birth certificate is sent to the office of vital statistics, where it will be registered with the state and permanently stored. Newborn birth certificates are available from the office of vital statistics no earlier than 10 days after the birth of the child.

Birth certificate records can be requested from the Office of Vital Statistics for the municipality where the baby was delivered (i.e. where the hospital is located). Learn more and see instructions >
